If you have a pet that has been seen at another veterinary hospital, it would be best if we transferred your pet's medical history to our hospital prior to your first visit with us. We want to make this process really easy for you!
Please download and complete a client record transfer form. By signing this form, you authorize our staff to handle the records transfer process for you. We will contact your prior veterinary clinic and let them know exactly what information we need. Because we will handle this for you, it is important that you sign this form and provide us with the signed document.
